The Licker is an abomination that underwent extreme mutations and bio-experimentation. As a result, it walks on four legs and has an extendable tongue, exposed brain, and no skin nor eyes. Prime 1 took on the daunting task of combining that grotesque and horrifying appearance with the lovely Cutie1 style. As you probably know, Cutie1 Plus figures share the important zipper. With a mouth like the Licker's, Prime 1 placed one well-opened to let its curvy tongue lash out, and since the Licker does not have eyes, they placed the well-known buttons on its shoulders.
